چکیده

The diagnosis and management of peripheral vascular disease has undergone tremendous change during the past two decades. Diagnostic angiography is complemented and, in some cases, even supplanted by digital subtraction techniques, duplex sonography, intravascular ultrasound, and magnetic resonance imaging. Similarly, percutaneous transluminal angioplasty has evolved from a novelty to a vital component in the treatment of peripheral vascular disease. This document discusses the background and current state of peripheral angioplasty, including the prevalence of peripheral atherosclerosis and the history, clinical results, and risks of angioplasty. As a less invasive treatment, angioplasty has been placed in an awkward position between surgery and nonoperative therapy. To clarify the role of angioplasty, the determinants of success, requisite symptoms for intervention, indications for treatment, and appropriate clinical setting are described. Because the alternative treatment is typically surgery when a patient's condition necessitates intervention, the literature comparing the two treatments is reviewed.

Acute aortoiliac occlusive disease during percutaneous transluminal angioplasty in the setting of ST-elevation myocardial infarction: a case report

BACKGROUND Aortoiliac occlusive disease, which is also referred to as Leriche syndrome, is a chronic atherosclerotic occlusive disease that occurs at the level of the aortic bifurcation. It is often thought to present with a triad of clinical symptoms: (1) intermittent lower extremity vascular claudication, (2) impotence, and (3) weak/absent femoral pulses. CASE PRESENTATION We report a case ...

The value of myocardial perfusion imaging with Tc-99m MIBI for the prediction of perfusion improvement after percutaneous transluminal coronary angioplasty

Introduction: Percutaneous transluminal coronary angioplasty (PTCA) is an effective method for revascularizing of stenotic coronary vessels. Lack of response to this treatment, either in symptomatic or asymptomatic patients, is usually due to incomplete revascularization, restenosis, and/or irreversibility of myocardial perfusion. Introduction of a noninvasive metho...

Role of Atherectomy Devices in the Treatment of Lower Extremity Peripheral Arterial Disease

Introduction Atherosclerosis is a systemic disease affecting all major vascular beds. Risk factors for atherosclerosis, and therefore, for PAD, include active smoking, advance age, dyslipidemia, diabetes mellitus, and hypertension. Due to continuous increase in aging population and diabetes, incidence of peripheral arterial disease will continue to increase. While atherosclerotic coronary disea...

When peripheral artery disease (PAD) affects the infrapopliteal vessels alone or in addition to the more proximal vessels, limb-threatening ischemia and tissue loss often occur. In the United States, between 150,000 and 300,000 cases of critical limb ischemia (CLI) are diagnosed each year.1 Within 1 year of diagnosis, 30% of CLI patients will have had a major amputation, and 25% will have died;...
